About the Rideau St. Lawrence Family Health Team

Established in 2019, the Rideau St. Lawrence Family Health Team (RSLFHT) is a community-focused primary care organization serving the municipalities of North Dundas, South Dundas, South Stormont, North Stormont, Osgoode and Russell.
The RSLFHT is a not-for-profit organization offering a range of comprehensive primary health promotion/chronic disease management programs and services through an inter-professional care team.
We currently collaborate with approx. 33 physicians and serve more than 46,000 patients.
Together, and in collaboration with the community, hospital(s), and other local health organizations, we provide integrated, team-based, patient-centered primary care through both direct office care, and virtual care visits.
Our purpose is to provide population-based centered comprehensive primary care services that are:
- Timely, coordinated and continuous
- Tailored to the needs of the community
- Support individuals to manage their own health & maintain independence.
- Build capacity in healthcare to facilitate the best possible healthcare outcomes for patients/families and communities.
- Collaborate with team-based Primary Care partner organizations across the Great River Ontario Health Team to provide comprehensive primary care to all patients of the region.
