Established in 2019, the Rideau St Lawrence Family Health Team (RSLFHT) is a  community-focused primary care organization serving the municipalities of North Dundas, South Dundas,  and South Stormont.

The RSLFHT is a not-for-profit Organization offering a range of comprehensive primary health promotion/chronic disease management programs and services through an inter-professional care team.

We currently collaborate with approx. 23-25 physicians and serve approx. > 39,000 patients.

Together, and in collaboration with the community, hospital(s), and other local health organizations, we provide integrated, team-based, patient-centered primary care through both direct office care, and virtual care visits.

Our purpose is to provide population-based centered comprehensive primary care services that are:

Tailored to the needs of the community

Support individuals to manage their own health & maintain independence.

Build capacity in healthcare to facilitate the best possible healthcare outcomes for patients/families and communities.
