You can access our services by self-referral or a referral from your physician. Please note: You must be a patient of the associated medical practices to be eligible for Team programs and services.

Chronic Disease Management

For those at risk or living with, lung disease, heart disease and/or diabetes. Goals include: maintaining or improving quality of life, reducing Emergency department visits and/or hospital admissions and increasing understanding of chronic disease management strategies and accessibility to specialized staff.

Health Prevention and Promotion

Preventative Care is the easiest way to be proactive about your health and protect your future! We offer Cancer screening, Flu Shots & Immunzations, and more.

Mental Health

Promote mental health awareness and illness prevention in the community and counselling of patients in an individual or group setting.

Pharmacy Services

Identify, prevent and resolve medication related problems; provide drug information and education.

Seniors Care

Provides predominantly home-based care and coordination of care for the most frail residents with multiple co-morbidities and higher socio economic need.

Access to Primary Care

Patients needing primary care service by nurse practitioner/physician assistant same day access.

Registered Nurses have a key role in the health care system. These professionals work closely with your family doctor and the other allied health professionals to navigate the patients’ healthcare needs.

Nurse Practitioner (s) & Physician Assistant

Nurse Practitioner (s) & Physician Assistant

These professionals possess advanced skills and work closely with your family doctor. The NP/PA are able to perform many different assessments, immunize and vaccinate, conduct physical examinations, order medical tests, diagnose & treat some illnesses, diseases, infections, injuries, and much more.



Our pharmacist is an expert when it comes to medication. He/She work(s) closely with family doctors and patients to provide insight and advice on medicine. The pharmacists are able to review the medication you take to ensure no medications oppose one another.

Providing patients with short-term counselling for issues such as anxiety, depression, or any psycho-social stress.
