Chronic Disease Management
Supporting individuals at risk or living with lung disease, heart disease, and diabetes. Our goals include maintaining or improving quality of life, reducing Emergency visits and hospital stays, and increasing access to specialized care.
Health Prevention & Promotion
Preventative care is a proactive way to protect your health and your future! We provide cancer screenings, flu shots, immunizations, and other preventive services to help you stay well.
Mental
Health
Promoting mental health awareness and illness prevention through community education and individual or group counseling to support emotional well-being and resilience.
Pharmacy Services
Addressing medication-related concerns through consultation, prevention, and resolution. We offer drug information and personalized education to ensure safe and effective treatment.
Primary Care
Providing same-day access to patients in need of care through nurse practitioners for timely and reliable healthcare support.
You can access our services by self-referral or a referral from your physician.

Mental Health Counsellors
Mental Health Counsellors offer short-term support for concerns like anxiety, depression, and stress. They provide compassionate counselling and strategies to help patients navigate psycho-social challenges.
Registered Nurses
