The Rideau St. Lawrence Family Health Team co-led an initiative uniting 4 organizations to co-design an innovative care model, breaking down silos between diƯerent team-based models and deploying co-located services in non-team-based models. Services are being embedded across all types of primary care models to enhance chronic disease programs, with plans for future expansion to other services. The collaboration dismantled siloes, prioritized equity and empowered providers across funding models. This scalable model demonstrates how diverse primary care organizations can jointly transform care, access, design, and delivery for rural, urban, and vulnerable populations.
